you to see any licensed physician
who can provide services to Medicare
beneficiaries and who is willing
to accept the terms and conditions
of the SmartValue plan. You are
not required to see a primary care
physician or to get a referral to
see a specialist. And now you have
more choices than ever. You may
select from four options: the SecurityChoice
Classic plan, SmartValue Plus plan,
which also includes Part D prescription
drug coverage, SmartValue Enhanced
and SmartValue Enhanced Plus which
also includes Part D prescription
drug coverage.
Monthly plan premiums range
from $0 to $98 depending on which
plan you select. (You must
continue to pay your Medicare Part
B premiums no matter which plan
you choose.)
