plan, you can enjoy the freedom and flexibility
of a traditional PPO plan with the added feature
of Self Directed Account (SDA).
The SDA is fully insured by PacifiCare Life
and Health Insurance Company and provides a
yearly credit of $1,000 for certain routine
and preventive services.
Any unused funds carry over year after year,
for as long as you are enrolled on the plan.
Once you deplete your SDA, or if you require
services not covered by the SDA, the plan defaults
to a PPO plan.
You must meet annual deductibles. Eligible medical
